Health Care Program

North Side Housing & Supportive Services provides on-site access to a nurse practitioner who addresses the physical health needs of Interim Housing consumers by providing them with routine physicals, treating minor health problems, monitoring chronic health problems (i.e. diabetes), prescribing and dispensing some medication and facilitating health care, prevention and wellness workshops.

The other components in our Health Care Program is the valued services of a Licensed Clinical Social Worker (LCSW) who delivers individual and group therapy mental-health sessions for consumers and supports them emotionally. Case managers round out the team, providing referrals that help build the participants personal support network, successfully integrate into community settings and encouraging personal growth, to ensure positive social health.

Our Health Care Program is currently open to participants in our Interim Housing and Permanent Supportive Housing Programs.
